The UFCW Unions and Employers Health and Welfare Plan of Central Ohio (the Plan) was established on January 1, 1956 to "provide for a uniform health and welfare program for employees employed under certain collective bargaining agreements" (excerpt from the Plan's Trust Agreement). The Plan provides health benefits to UFCW members througout central, northern and southern Ohio. The Plan exists as a result of collective bargaining agreements between UFCW Local 1059 and various participating employers. The list of participating employers in the Plan is as follows:- CVS

Each of these employers has agreed (through the collective bargaining process) to make contributions to the Plan on behalf of its eligible employees so that the eligible employees have health benefits. These benefits may include medical, dental, vision, life insurance and prescription drug coverage to name a few.
